Call during office hour, email, or fax to obtain the Return Merchandise Authorization number (RMA #). Please provide Customer Name, Invoice Number, Product Item and quantity, Reason for return.
Return your product using the appropriate shipping service with tracking so you may have receipt of the product being returned. Remember to include RMA # and return address. All product return fees are the responsibility of the customer unless the return is the result of an error by CLK Medical Supply.
All returns must have RMA#. Returnable products may be returned up to 30 days after delivery.
All parts and accessories must be included and in original packaging.
Shipping Fees are non-refundable and return shipping is the responsibility of the customer.
If your orders shipped for free and you are returning it we will deduct the original outbound shipping costs from your refund.
If your return is the result of a defective product or shipping error, CLK Medical Supply will refund the entire purchase amount and return shipping costs.
Some products are Non-Returnable. Opened items, Special orders, Custom orders, all clearance items, bulk orders, and used or tried-on items.
If you paid for the purchase with a credit or debit card, your account will be credited back the amount of the purchase less any shipping charges within fifteen (15) business days from the time CLK Medical Supply receives the returned product(s) or we will issued credit into your account for your next purchase.
Any product shortage must be reported within 48 hours from receipt of the order at your location. You must count your item(s) upon arrival and document any shortages on your delivery receipt.
